BENEFITS CHECKLIST

-If you have out of network benefits through your medical insurance and you would like to submit for potential reimbursement, I will give you an itemized bill after each session (PT evaluation/treatment only) and you can submit this to your insurance company, flexible spending account or health savings account.  

-If you have insurance but are not sure if you have out of network benefits, I have created a "benefits script" you can use when calling your insurance.  The script will help you know what questions to ask your insurance.
